Six Painful Reasons for Initiating Organization
Medication Misdispensing
A physician prescribed 8 bottles of IV Albumin solution for an ICU patient but patient received 8 bottles of IV immunoglobulin solution due to dispensing error (Look-Alike medication) and administrational error.
Life-Threatening
A physician prescribed FLUVIR (oseltamivir) tablet for a small girl to treat flu but the child consumed adult dose of STAVIR (stavudine) tablet which is used for the treatment of HIV infection.
Nursing negligence
Many of the hospital do not recruit qualified nurse and due to this, patients are exposed to significant risk of infection. You can see many cases where unqualified Nurse giving injections to the patient in an unsterile manner.
Medical malpractice
The patient consulted two physicians, receiving prescriptions with the same active ingredients for asthma, weakness, and high blood pressure. Taking both sets of medications for years led to adverse reactions and death. The prescribing physicians were unaware of the error, attributing it to the patient's innocence or mistake.
Manufacturing Oversight
Many patient and physician have noticed "Lack of Drug Effect". Reasons are plenty but you could not underestimate the incorrect manufacture &/or its storage reason (as certain product such as enzymatic preparation, insulin etc. need special storage).
Cross-sector contamination
In some manufacturing industries, non-pharma staff are involved in drug production, leading to numerous unintentional errors. They often lack knowledge of critical factors like sterility, temperature's impact on enzymes, sampling techniques, vessel calibration, and foreign particle effects in IV products.
